When you send an enquiry through this website we collect your name, phone number, email address, the patient's age, your preferred contact method, and the reason for your enquiry. This information is used only to respond to your enquiry and, where you choose to proceed, to arrange a consultation.
The reason for your enquiry may include information about health. Health information is special category data under UK data protection law, and it is processed on the basis of your explicit consent, which the enquiry form asks for before you submit. Please keep medical detail in the form to a minimum — a full history is taken at consultation.
For other enquiry details, our lawful basis is taking steps at your request prior to entering into a contract of care, and our legitimate interest in responding to people who contact the practice.
If your enquiry does not lead to a consultation, it is deleted within 12 months. If you become a patient, the information you provided forms part of the medical record and is retained in line with the retention periods that apply to healthcare records.
Enquiries are delivered to the practice by our email service provider, which processes them on our behalf. Your information is not shared with anyone else and is never used for marketing. This website does not use analytics or advertising cookies.
You have the right to ask for a copy of the information we hold about you, to have it corrected or deleted, to restrict or object to its use, and to withdraw your consent at any time. To exercise any of these rights, contact the practice at enquiries@example.com.
If you are unhappy with how your information has been handled, you have the right to complain to the Information Commissioner's Office at ico.org.uk.
